The "Gut Brain" and Digestive Health
The digestive tract is an exquisitely complex interface between diet and health. Widely recognized approaches to digestive support include probiotics, prebiotics and enzymes. However, the greatest scientific and clinical advances in gastroenterology over the past two decades have addressed the functional aspect of digestion-GI motility. GI motility refers to peristalsis, the smooth muscle contractions that propel digestive contents along the GI tract. These rhythms are critical during and after meals to maximize contact of nutrients with villous surface area for absorption. they are also important between meals to clear residual food and debris, supporting a healthy microflora population. Motility is controlled by the energetic nervous system (ENS), or "gut brain", a network of over 100 million neurons embedded in the lining of the GI tract. Although the ENS is influenced by the central brain, it functions independently as a branch of the autonomic nervous system. The neurons within the ENS communicate using many of their local roles in the GI tract are to regulate smooth muscle activity. The most important regulators of the digestive motility are serotonin and acetylcholine. Over 95% of the body's total serotonin is generated by the GI tract, where it orchestrates motility by binding to serotonin type 4 (5HT4) receptors. (1,2) Once activated, these receptors stimulate the realease of acetylcholine into neuromuscular junctions, resulting in coordinated peristalis. (1,2) Both 5-HTP and ginger supposrt coordinated motility of the upper and lower GI tract, in part by supporting 5-HT4 activity. (2,4) Ginger also acts to involve the serotonin type 3 (5HT3) receptor, partially explaining its utility in helping to relieve occasional nausea. (4) Acetyl L-carnitine provides general supposrt for the health of motor and sensory neurons of the peripheral nervous system, including the ENS. While promoting gastrointestinal motility is valuable in many clinical contexts, maintaining calming GI rhythms is sometimes desirable. Melatonin supplementation supports this action in the distal small intestine and colon, in addition to protecting the mucosa and maintaining inflammatory balance and regenerative capacity of the epithelia. (3,5) The triterpenoid AKBA from Boswellia serrata and the flavone quercetin have similar mechanisms which may support tempered contractile patterns of ileal and colonic smooth muscle. (6,7) Recent studies indicate curcumin and boswellia offer support for cytokine balance and epithelial turnover as well. (8) Additional research is necessary to clearly characterize the potential digestive utility of these nutrients. The progressive scientific and clinical convergence of neurology and gastroenterology will likely open new doors to individualized management of digestive health.

Higher Protein Low-Glycemic Diets are Key for Weight Maintenance
A recent article published in the November New England Journal of Medicine adds more evidence that a high protein, low glycemic index diet may be the key for weight management and maintenance. The study involved over 500 overweight adults from eight European countries. All participants had initially lost at least 8% of their total body weight on an 800-1000 calorie per day diet, and were then randomized to a low protein (13%) or high protein (25%) diet with a low glycemic or high glycemic index. All of the diets in the study had a moderate fat content (25-30% of total) with no restriction on total calories. Results indicate that both the high protein and low glycemic index diets offer statistically significant support for weight maintenance. Participants following the high protein diet had an average of 2 pounds less weight regain than those following the low-protein diet. The low-glycemic diet had similar findings. Additionally, the high-protein groups and low-glycemic index groups were both more likely to achieve additional weight loss during the maintenance phase. No differences were reported in appetite between the groups, however it may be that the effects were too subtle to be accurately measured. This research mirrors similar findings for weight management. In a 2006 meta-analysis, diets consisting of <35-41% energy from carbohydrates and > 1.05 grams of protein per kilogram body weight were associated with healthy weight management and body composition. A meta-analysis of six randomized controlled trials showed similar support for weight management and body composistion with low-glycemic index diets, as well as support for lipid metabolism. These results even extended to comparisons of ad libitum low-glycemic diets with conventional restricted energy low-fat diets. Ensuring protein and fiber intake (lower glycemic index) can be an important part of any weight management or maintenance plan. These nutrients allow for a slower release of glucose into the bloodstream, promoting glucose metabolism. Emerging research suggests that this support may be key to the cardiovascular, cellular and metabolic health benefits offered by a low glycemic diet.

The Following are conference Highlights: *The goal of testosterone replacement is to restore levels to a healthy physiological range. *Men with benign prostatic enlargement,(BPE), must be careful if using Proscar as it can lead to suicide after discontinuation as well as more aggressive types of Prostate Cancer. These are two reasons we prefer to try PROSTATHERA as an initial treatment for BPE. *When taking testosterone replacement we aim to keep the estrodiol & sex hormone binding globulin levels mid-range. High estrodiol can lead to impotence, edema and increased female fat distribution. *HCG (human chorionic gonadotropin) is an adjunct to testosterone as it can allow for a lower dose of testosterone to be used and preserve fertility & testicular architecture. *To minimize side effects from testosterone gels one should use the smallest amount of product to a limited area of the body. *PREGNENOLONE can be used for the treatment of anxiety, depression and memory impairment. It functions as a neuro-transmitter in the brain's hippo-campus. *The symptoms of low thyroid function that respond to thyroid Rx include: fatigue, depression, weight gain, dry skin, brittle nails, thinning hair, cognitive dysfunction, constipation & hyperlipidemia. *Fibromyalgia may be due in part to Thyroid hormone resistance (cellular hypofunction), hence these patients typically require higher doses of thyroid replacement. *Growth hormone levels increase in response to aerobic training. *Women get an increase in DHEA levels after exercise. *People with high levels of job stress where they have a low level of control have High ratios of Cortisol/Dhea. In men this suppresses testosterone levles and increases the risk of metabolic syndrome. *Increased stress has been associated with an increased risk of breast cancer. *Diabetes, hypertension and heart disease are responsible for most sexual disorders. *HGH (human growth hormone) suppression is associated with cancer. *HGHdosage requirements are typically higher in females. *Adrenal Fatigue is most often caused by chronic stress/anxiety and depression. In stage 1: Cortisol levels are increased and DHEA may be normal or increased. In stage 2: Cortisol is high and Dhea is low. In stage 3: Both Cortisol and Dhea levels are decreased. Severe adrenal fatigue can take 12-24 months to resolve with appropriate treatment. *New research into BREAST CANCER is revealing that the EXTRACELLULAR MATRIX (the web holding cells in place) may be a promotor of cancer if it becomes de-stabilized by INFLAMMATION. To stabilize the matrix you must decrease tissue inflammation by using green tea extract, genestin, resveratrol, detoxification and hormone balancing. *Fat Loss Accelerators include: 1. using Protein meal replacements 2. remove high reactive foods 3. increase water and fiber (aim for 50 grams of fiber per day) 4. restrict carbs (starch, sweets, fruits) 5. utilize BURST-STYLE training-go all out for 30-60 seconds, then recover for 60-120 seconds. Repeat this sequence 4 times. *120 genes have been identified that are involved in human obesity. Genetic factors are estimated to account for 40-70% of OBESITY. Using genetic testing helps to change eating patterns & customizes each person's weight loss and exercise program.

These are the highlights of key points of the Sept. 23 & 24 symposium I recently moderated and attended in Alexandria, Virginia: *Low levels of BIFIDO-BACTERIA in infancy lead to obesity later in life. *Microbes prefer living in the gut attached in collaborative microbial communities in BIO-FILMS, Also common bio-films would include dental plaque. Bio-films allow for microbes to encase themselves in a self-produced MATRIX. This BIO-FILM protects against predators, competitors, toxins, and antibiotics. *Bio-film-associated human infections include: Chronic sinusitis, breast implant infection, infected burns, chronic ear infections,and prosthetic joint infections. *Gut Bio-film alteration can lead to: deranged immune infection, impaired G.I. motility & function, decreased toxin clearance and increased inflammation. *10-20% failure rate for H. Pylori is due to Bio-film that is Urease+ *Chronic fatigue syndrome & fibromyalgia is associated with disrupted G.I. microflora: lower bifido-bacterium, increased enterococcus, decreased E. Coli, increased candida albicans in acute phase. Here bio-film may explain the high recurrence rate after treatment for yeast. Therefore, treatment with digestive enzymes, chelating agents, pro-biotics & pre-biotics may be helpful. *By young adult hood humans have over 100 trillion bacteria in their intestines. This is 10X the number of cells than in the entire human body. *Healthy bacterial flora are involved in metabolism, immuno-modulation and pathogen protection, colonization begins at birth & with breast feeding and continues through life. *Children who are "too clean" are more likely to develop allergies and asthma. *50% of the patients with Irritable Bowel Syndrome respond to Pro-biotics. Patients with inflammatory bowel disease need the HIGHEST DOSES of PRO-BIOTICS. *Dysbiosis is a breakdown in the lining of the GI tract leading to increased inflammation & increased dysfunction of the immune response. Dysbiosis can be caused by: broad spectrum anti-biotics, acid blockers, chronic constipation, stress, anger or fear. *To prevent antibiotic induced diarrhea one should initiate probiotic therapy as soon as antibiotics are begun. *GI disturbances in AUTISM show the following clues: difficulty breast feeding, persistent colic, GE reflux, infantile eczema, food sensitivities, failure to thrive, history of frequent antibiotic usage, abnormal posturing, self-injurious behavior and poor sleep. *Stool patterns of children in the autism spectrum reveal: diarrhea, constipation or alternating diarrhea and constipation. *When looking at autistic children's CYTOKINE PROFILES (immune system chemicals) in the DUODENUM we see that they have high levels of inflammatory cytokines and low levels of anti-inflammatory cytokines. *A healthy diet for autistic children would EXCLUDE: yeast promoting foods, aspartame (excitatory neurotopin), MSG, dyes and additives. It would INCLUDE: probiotics, digestive enzymes and Omega 3 Fatty Acids. *Fructose should be avoided as it turns into free fatty acids, VLDL (damaging form of cholesterol) and triglycerides. *Autistic children have germs that are out of balance, in particular many subtypes of clostridium difficile as well as fungus and parasites such as blastocystis hominis. *Autistic children need to be supplemented with: vitamins A, D, E, K & B vitamin family, zinc, magnesium and calcium. *Probiotics have a role in celiac disease. *Natural antimicrobials include: BERBERINE, OIL of OREGANO and CAPRYLIC ACID. *Periodontal disease has been linked to atherosclerosis. *Apthous ulcers have been postulated to be a marker for some auto-immune disorders. *Avoid antimicrobial mouthwash as it disrupts the normal balance of oral flora. *In Irritable Bowel Syndrome the GI dysfunction may not just be gut related because of the gut-brain axis. *The GI microbiota plays an important role in maintaining normal intestinal function. Altering this microbiota can lead to significant changes in gut function. *Intestinal infection can lead to a significant and long-term activation of the enteric immuno-endocrine system. *Probiotics have a place in the RX of IBS but more studies need to be done. *For acute gastroenteritis in children a combination of saccharomyces boulardii and lactobacillus GG has been shown to be effective. *Antibiotic associated diarrhea usually starts 5 Days after starting antibiotics, but it can also occur up to 8 weeks after starting antibiotics. *S. boulardi has been shown to help eradicate heliobacter pylori (causes gatritis, ulcers) and help eradicate candida in autistic children. *Probiotics should not be given to patients with ACUTE Pancreatitis. *Human breast milk contains the mother's microflora and can transfer it to her newborn during breast feeding. *E. Coli Nissle 1917 bifidobacterium infantis and S. Boulardi are beneficial bacterial organisms for people suffering from Crohns disease. *For probiotics to be effective in inducing clinical changes they must be taken on a long term basis. *Probiotics have been shown to reduce the symptoms of allergic rhinitis, asthma and atopic dermatitis. *Always take a probiotic with food because the pH of your stomach will be between 4-7.

Study shows water consumption increases weight loss during diet interventio
STUDY SHOWS WATER CONSUMPTION INCREASES WEIGHT LOSS DURING DIET INTERVENTION Subjects: 18 men and women, 55-75 years old, Body Mass Index 25-40 Dieters drinking 16oz of water prior to each meal increased their weight loss by 4.4 pounds and fat loss by 4.6 pounds in 3 months. The authors of this new study stated that their results indicate that when combined with a hypocaloric (low calorie) diet, consuming 500 ml (approx. 16 fl oz) of water 30 mins prior to each of three main meals daily, leads to a greater weight loss over 12 weeks compared to a hypocaloric diet alone. "This difference was attributed to a 44% greater rate of weight loss among water group participants compared to non-water participants over the 12-week period," they stated. Those in the water group lost an average of 11.9 pounds of body fat versus 7.3 pounds for those in the non-water group. Water reduces the calorie intake, although it is not clear how long this effect lasts. "This effect may be due in part to an acute reduction in meal (energy) intake following water ingestion, which we observed at the baseline laboratory test meal studies." A reduction in meal intake following water consumption is accompanied by increased sensations of fullness, which may facilitate a lower meal (energy) intake following water ingestion. Conclusion: Drinking 2 cups of water before meals increases weight loss when combined with a low calorie diet. Dieters drinking 16oz of water prior to each meal reduced intake by roughly 225 calories a day.

Use of Meal replacement Products Increase Weight Loss
A recent article published in Obesity 2009 showed that the most important predictors of successful weight reduction were found to be the number of follow-up visits attended, use of meal replacement products and minutes of weekly activity. An article in Diabetes Care 2007 also showed that using protein meal replacements increase weight loss. These products make it easier to cut calories. In light of such evidence we at Capital Region Progressive Medicine strive to present our patients with a wide range of protein meal replacements to fit everyone's taste buds and budget. In fact in some cases these protein meal replacements are less expensive than other supermarket based alternatives.

WEIGHT LOSS ALERT!!
A recent study published in the American Journal of Medicine reported that compared with age and gender-matched SUICIDE RATES in the U.S. There was a substantial EXCESS OF SUICIDES among ALL PATIENTS undergoing bariatric surgery in Pennsylvania during a 10 year period. 30% of suicides occured within the first 2 years following surgery, with almost 70% occuring within 3 years. Suicicide rates were typically higher among men than women. This information documents the need to develop more comprehensive longer-term surveillance and follow-up methods in order to evaluate factors assocoiated with post-bariatric surgery SUICIDE. These suicides may be related to myriad factors. I recommend that anyone considering BARIATRIC SURGERY think long and hard about the pros & cons of this surgery.

Integrative Medicine Preceptorship
Dr. Erner has developed a program in conjunction with Integrative Therapeutic, Inc. ( one of the largest nutritional/herbal supplement companies in the U.S.) to train interested health care practitioners in Integrative Medicine. The candidates can spend Thursdays and Fridays in our office seeing patients with Dr. Erner. In this clinical setting they are able to see the true appllication of integrative medical principles by interacting with our patients, reviewing their medical information,etc. Thus far we've had several HCPs visit our practice and all enjoyed the experience immensely. As part of this program, Dr. Erner has been named to Integrative Therapeutic's medical advisory board and will be periodically be participating in medical conference presentations when the company is an exhibitor.
